This page is designed to expedite your wisdom teeth consultation appointment.

The first thing you should do, is download the registration forms if you haven't done so and complete them. You can get them by clicking the "download" button below. Bring them completed to your appointment.

Next, follow the link by clicking on the picture below and view the video on wisdom teeth. Once the new page loads, click on the "click to start" button for the video to begin.

After you have finished watching the video, please download the "Video Consent Questionnaire". You can get that by clicking the "Get It Now" button below. Complete the questionnaire, sign it (if you're less than 18 years old a parent or guardian must sign the form) and bring the questionnaire with your registration form to your consultation appointment. If you are less than 18, a parent or legal guardian must accompany you to your consultation appointment and your surgical appointment if one is made.

You should also review the post-operative instructions and video. The post-operative video available by clicking the "OK" button below. The post-operative instructions are available here or at the bottom of the page.

Once you hit "OK" you'll be taken to a page that looks like the image below. The video to watch for the post-operative instructions is the one below (the area where the arrow is pointing).

After your follow-up visit you usually will be asked to irrigate the lower surgical sites beginning about 7 days after your surgery. Post-operative syringe instructions can be found below as well as a demonstration video.
